Moving with a 15 year old cat.?

I have a 15 year old cat that I may have to take with me when I move. At the moment, he's staying with my father. But he's not sure if he can keep the cat. He's still very healthy and acts like he's a kitten. Shows no signs of old age. But my only concern is, I'm moving 12 hours away. I fear the drive will freak him out since I know he does hate cars (I think mainly because the only time he's been in one is to go to the vets). Are there any precautions I should know about? Things I may need to do before the move or take with me?
